The relevance of the study is driven by the launch of new Russian national healthcare projects in 2025, aimed at addressing systemic issues in primary care, which is crucial for ensuring healthcare accessibility for the majority of the population. The purpose is to analyze the effectiveness of the national projects' implementation in primary healthcare, identify key challenges, and compare the situation with international experience. The methodology is based on a comparative analysis of statistical data (from the Ministry of Health, Rosstat, the Accounts Chamber) for Russia and European countries, alongside a case study of the North Ossetia region as a problematic example. The results revealed that despite the modernization of 1,850 clinics and the opening of 4,200 FAPs (feldsher-midwife points), profound inequality in access persists (30% of rural residents do not use online booking), alongside a personnel shortage (a deficit of 23,000 doctors in rural areas) and inefficient use of funds. A comparative analysis with leading European healthcare systems (Germany, France, and Sweden) demonstrates a significant gap in the efficiency of medical processes and the depth of digital transformation in domestic healthcare facilities. The conclusions emphasize that achieving the national projects' goals by 2030 requires a shift from quantitative metrics to qualitative ones: overcoming regional disparities, unifying digital systems, and solving the problem of retaining medical personnel in rural areas.
